About VISTE:
The mission of VISTE is to enable frail elderly persons to live independently and safely in their own home.

Position Summary:
The Driver is responsible for transporting clients in a safe, courteous, timely, and professional manner.  The specifics are determined by the varying needs of the clients, as assigned by the Transportation Manager.

Position Eligibility:

  1. Strong organizational skills

  3. Basic map reading skills or ability to safely use navigational phone applications
  4. Knowledge of Polk County geographical area, roads, and locations
  5. Ability to communicate effectively
  6. Ability to manage a schedule
  7. Willingness to work beyond standard hours, as needed
  8. Valid Florida Driver’s License
  9. Valid auto insurance
  10. Ability to drive in all reasonable conditions (varied weather, non-daylight hours, etc)
  11. Access to a vehicle in good repair
  12. Physically able to complete assigned tasks
  13. Ability to lift up to forty (40) pounds
  14. A high school diploma or GED, and three or more years of relevant experience, or an equivalent combination of education, training, and experience to provide the necessary knowledge, skills, and abilities necessary for success 

Position Duties:
In order to support VISTE in the mission of enabling frail elderly persons to live independently and safely in their own homes, the Driver will do the following.

  1. Transport clients to their appointments as assigned by Transportation Manager.
  2. Assist clients as necessary.
  3. Communicate client needs or concerns to the appropriate VISTE office.

Position Standards:
It will be known that the person holding this position is successfully completing duties and performing tasks when the following are accomplished.

  1. Clients are transported to their appointments in a safe and timely manner.
  2. Information about client needs is communicated to relevant VISTE departments.
  3. Clients are able to remain safely in their homes.

Position Tasks:

  1. Work with the Transportation Office to know when client transportation is scheduled.
  2. Call clients 24 hours in advance of the appointment to confirm transportation needs.
  3. Pick up clients at the agreed upon time, at their door even if prior contact has not been established. Pick up time is typically 30 minutes prior to appointment time.
  4. Assist client into and out of the vehicle as necessary.
  5. Assist client with carrying personal items (groceries, walker, oxygen, etc.) based on the individual needs of the client.
  6. Return client to their home or final destination.
  7. Remain in contact with the Transportation Department in order to learn of and communicate about any changes or concerns.
  8. Use the most efficient manner of travel and mileage.
  9. Utilize approved identification to show the client and community that the transportation is provided by VISTE.
  10. Record and submit mileage and time as required by the Finance Director.
  11. Submit daily paperwork as required by the Transportation Manager.
  12. Follow all rules and guidelines established by law, VISTE policy, and the Transportation Manager.
  13. When necessary, share any safety, health, repair, or personal needs of clients by use of the incident report submitted to Transportation Manager.
  14. Maintain vehicle used for client transportation. Vehicle must remain safe, clean, organized, and accessible to clients.

Position Summary:
The Client Case Coordinator is responsible for serving as the liaison between clients, potential clients, and other VISTE departments and outside agencies that can best serve their needs.

Position Eligibility:

Education and Experience

  1. Ability to communicate effectively.
  2. Strong organizational skills.
  3. A working knowledge of and experience with Microsoft Office Suite, including Word, Excel, and Outlook.
  4. Willingness to work on tasks as assigned.
  5. Willingness to work beyond standard hours, as needed.
  6. Ability to maintain confidentiality related to client and donor information.
  7. Capacity to work independently and willingness to help outside your job scope when necessary.
  8. Valid Florida Driver’s License and auto insurance.
  9. Bilingual, English/Spanish, preferred
  10. High school diploma or GED, and minimum 1 year of customer service experience, or an equivalent combination of education, training, and experience to provide the knowledge, skills, and abilities necessary for success.

Position Duties:
In order to support VISTE in the mission of enabling frail elderly persons to live independently and safely in their own homes, the Client Case Coordinator will do the following:

  1. Job Knowledge:
    • Develop a complete understanding of the programs and services offered by VISTE.
    • Conduct interviews and evaluate potential clients.
    • Compose clear, direct, concise, and complete client intakes and requests for services.
  1. Communication:
    • Clearly communicate VISTE’s mission to potential clients and stakeholders.
    • Communicate with clients and potential clients in a clear, patient, and respectful way.
    • Clearly communicate procedures and standards of VISTE’s intake process and client requests.
    • Keep Client Case Manager informed about progress and problems.
  1. Teamwork:
    • Listen and respond appropriately to other team member’s ideas.
    • Give honest and constructive feedback and show willingness to receive feedback from supervisors and peers.
    • Provide assistance to team members when needed.
  1. Accountability:
    • Show up to work on time, and follow instructions, policies, and procedures.

Position Standards:

  1. Information regarding clients and services is maintained accurately and regularly.
  2. Potential clients understand the scope of services VISTE can provide that best suit their needs.
  3. The needs of clients are understood, and services and referrals are provided when appropriate.
  4. Ensuring potential clients complete the intake process in a timely and courteous manner.

Position Tasks:

  1. Conduct and coordinate client intake interviews in their home or over the phone.
  2. Inform other VISTE staff and departments of services needed by clients.
  3. Assure that all required client paperwork and forms are completed, updated, and accurate.
  4. Remain in contact with clients as needed, and as client needs change.
  5. When appropriate, refer clients to other community resources.
  6. When appropriate, coordinate services with community resources and agencies.
  7. Support and provide guidance for intake volunteers and oversee client documentation for completeness and accuracy.
  8. Maintain client database accuracy.
  9. Complete weekly administrative tasks related to data entry and file maintenance.
  10. Perform related work as needed.
